Arizona’s State Plan on Aging survey

Arizona’s State Plan on Aging survey

Every four years the State of Arizona is required by the Older Americans Act, to develop a strategic plan for its aging citizens, in order to receive funding for the vital local services, such as transportation, elder abuse, the Ombudsman program and other programs covered under that Act.

Therefore, as part of the needs assessment gathering process, information gathered by this survey, along with information gathered in other forums, will provide vital insight to identifying  the needs and issues facing aging Arizonans and their caregivers, moving forward for the years 2019 through 2022.

The survey should only take about 5 minutes to complete this survey.

Please share the link https://www.surveymonkey.com/r/State_Plan_on_Aging  with individuals or any collaborating, partner agencies that you may be affiliated with, work with regularly, or know would be a good resource to get this circulated through.

The survey will accept responses from October 25, 2017 through December 15, 2017.
